Welcome to our online booking site! Please note: If you are booking your first appointment, you will be prompted in your booking confirmation to fill out your forms online. If you cannot complete your forms at the time of booking, you will receive an email with the forms, as well. We ask that you complete your forms and provide any applicable insurance information a minimum of 24 hours prior your appointment so we can give you the best experience possible! When you arrive for your appointment, you can find convenient, designated patient parking by Door #1 (main entrance) for the Bloomington Clinic and Door #10 for the De Rusha (Student Teaching) Clinic We look forward to seeing you soon!
